This page has links to the forms you need to request a new P-Card or Meeting Card or to confirm you received a physical card. If you have questions about filling out these forms, contact the Card Services team or your local business office.
- P-Card & Meeting Card Request Form: Use this form to request a new P-Card or Meeting Card. The P-Card Essentials course must be completed by the responsible cardholder prior to applying for a card. There is currently no training requirement for Meeting Cards.
- P-Card & Meeting Card Receipt Acknowledgement Form: Submit this form once you receive your new P-Card or Meeting Card.
